Recognizing Perimenopause and Menopause

Around 1.3 million women enter menopause each year in the United States. The average age of onset is 51, but perimenopause can begin in the mid-40s and last 4 to 8 years. About 85% of women experience at least one disruptive symptom.

These symptoms are driven by declining levels of estrogen, progesterone, and testosterone that affect nearly every system in your body. The good news: they're treatable.

Common Symptoms & How Often They Occur

Hot flashes and night sweats 75-80%
Sleep disturbances and insomnia 40-60%
Brain fog and memory issues 35-60%
Mood changes, anxiety, and irritability 40-50%
Vaginal dryness and painful intercourse 40-55%
Weight gain and metabolic slowdown 30-50%
Fatigue and low energy 40-60%
Decreased libido 20-40%
Joint pain and muscle aches 40-50%
Hair thinning 20-30%

Hot flashes last an average of 7.4 years. (SWAN Study, 2015)

Only about 25% of symptomatic women seek treatment. You don't have to be one of the 75% who suffer in silence.

What is Bioidentical Hormone Therapy?

Bioidentical hormones are structurally identical to the hormones your body naturally produces. Unlike synthetic hormones (such as conjugated equine estrogens or medroxyprogesterone), bioidentical formulations like estradiol and micronized progesterone are designed to work with your body's existing receptor systems.

The landmark 2002 Women's Health Initiative study raised concerns about hormone therapy, but those findings were specific to synthetic formulations (Premarin + Provera). Subsequent research, including the E3N French cohort study, found that women using micronized progesterone showed no increased breast cancer risk, unlike those on synthetic progestins.

HRT has been shown to reduce hot flash frequency by 75-95% and significantly improve sleep quality, mood stability, and vaginal health according to Cochrane Reviews.

Bioidentical vs. Synthetic

Bioidentical (What We Use)

  • Molecularly identical to human hormones
  • Derived from plant sources (soy, yams), then processed to match
  • Can be customized to individual needs
  • Favorable safety profile in recent studies
  • Multiple delivery methods: pellets, creams, patches, oral

Synthetic (Traditional HRT)

  • Different molecular structure than human hormones
  • One-size-fits-all dosing
  • Associated with increased risks in the WHI study
  • Premarin derived from pregnant horse urine

Frequently Asked Questions About BHRT

Is bioidentical hormone therapy safe?

When prescribed and monitored by a qualified physician, BHRT has a favorable safety profile. The KEEPS trial and E3N cohort study both showed positive outcomes with bioidentical formulations. Dr. Strand monitors your levels regularly and adjusts your protocol to minimize risk and maximize benefit.

How is BHRT different from traditional HRT?

Bioidentical hormones are molecularly identical to what your body produces naturally. Traditional HRT often uses synthetic hormones like conjugated equine estrogens (Premarin) and medroxyprogesterone (Provera), which have different molecular structures and were the formulations studied in the 2002 WHI trial that raised safety concerns.

How quickly will I notice results?

Many women notice improvements in sleep, mood, and hot flashes within 2-4 weeks. Full optimization typically takes 2-3 months as we dial in your dosage based on follow-up labs and symptom tracking.

What hormones are included in a BHRT protocol?

Depending on your lab results and symptoms, your protocol may include estradiol, progesterone, testosterone, and DHEA. Each is dosed individually based on your unique biochemistry.

Can you treat perimenopause, or only full menopause?

Yes. Perimenopause, the transitional period that can begin years before your last period, is when many of the most disruptive symptoms start. We treat both perimenopause and menopause with personalized protocols.

Do you offer telehealth for menopause care?

Yes. All consultations and follow-ups are available via telehealth for patients anywhere in Washington state. Many women find this more convenient for ongoing care.

Does insurance cover BHRT?

We are a direct-pay practice and do not bill insurance. Our evaluation and membership pricing is transparent and often comparable to traditional copays and deductibles. Many patients find the quality of care and time spent with their provider far exceeds what insurance-based practices offer.
